That was a doc that was graciously given a copy of to me by an excellent
virtual friend, that I sent to some other great guys back 4 or more years
ago (our man Bruce, Mike G, Mike V, others), it was my thank you to them for
the help they provided in areas I was learning in.  I think copies of it
were going around before but more underground.  Read and read it again (good
stuff but watch for missing sentences, major typos and such), then wish you
had the other areas that are missing, of which if I take a flight to another
new acquaintance, will be able to see and maybe copy the missing parts
(boost
control etc), when I get caught up with all my other commitments to people
:-).  From what I understand, all the remaining is close to 2,000 pages
long,
time to take a sabbatical to Aruba and just spend a few months absorbing sun
and all this great information ;-)

I never needed to investigate this so someone else will have to fill in
here, but after you read the rest, may understand more.
