Mike,

Basically, if the O2 doesn't switch rich/lean, code 13 sets within a minute
of driving.

For a code 13 you need engine at operating temp, at least 2 minutes engine
time running, O2 signal basically steady between 0.35 and 0.55 volts, TPS
above 4%.
All conditions must be met for at least 60 seconds. Should be in closed
loop--but if code 13 sets, it never goes closed.
Check voltage at the O2 sensor--should be biased at 450mV to ground. (O2
disconnected-KOEO). With a scan tool connected, jumper the O2 lead to
ground -- you should have less than 200mV O2 signal on the scan tool engine
running. If the code still sets and all voltages are good, you've got a bad
ECM. Check for a shorted lead to the O2 (to ground) from the ECM. Hope this
