I've never done anything with TBI,  but with respect to puddling what/how
are you mounting the TBI to the intake?   I see you swapped back to a 4bbl
- didn't say if you changed the intake or not.  

If it's adapted to a 4bbl intake, that might be one source of the problem.
 How about trying a bone-stock TBI intake?   Yeah, I've heard that they're
not much good past 4k, but you should at least see SOME difference which
might point in the right direction.  

Any chance the EGR valve (if it's still there) is somehow opening?   

How about where the MAP sensor is sourced?   Mabye that location is getting
some sort of reversion in/from a runner at that RPM and causing the ECM to
lean it out.

These are the last oddball things I can think of.
